For salaried borrowers:

| _____ | Home insurance premium for home & investment properties (if applicable). |
| _____ | Most recent mortgage statement for home & investment properties (if applicable). |
| _____ | Homeowner Association (HOA) payment coupon for home & investment properties (if applicable). |
For self-employed borrowers:
| _____ | Copy of last 2 years' Federal 1040 Tax Return with all schedules. |
